The Preparation Checklist outlines all the information you need to bring to your appointment. By printing this checklist and bringing all the requested materials, you can help us provide you with the best medical care.

The Screening Questionnaire may also be printed and completed prior to your visit. Determining your current immunization needs is important so we ask that you bring the documentation of your immunization history with you to the visit.

Be sure to call as well in advance of your departure date as possible, at least six weeks prior to your trip, as some of the immunizations must be given in sequence over several weeks. If you are traveling for more than six months or are relocating to another country, we encourage you to visit us several months prior to your departure.

Cost and Payment

Costs will vary depending on the specific services you receive. There is a fee for the visit and a fee for each immunization you receive. We ask for complete payment at the time of service. Although insurance generally does not reimburse for costs associated with vacation travel, we will provide a comprehensive itemized bill for you to submit to your insurance company.
